AECSD Parent/Guardian - Pfizer/BioNTech Vaccine Sign-Up for Students Ages 5-11

UPDATE: November 18, 2021

For students ages 5 through 11 that received the COVID-19 vaccine in our schools on November 16 and November 18, the Cayuga County Health Department will be returning to administer SECOND DOSES to those students on December 7 and December 9.

For any parents/guardians who missed the sign-up or did not return the consent and medical screening forms in time for their child ages 5 through 11 to receive the COVID-19 vaccine at the clinics on November 16 and November 18, the Cayuga County Health Department will also offer FIRST DOSES of the COVID-19 vaccine when they return to our schools on December 7 and December 9. See below for instructions on signing up and returning consent and screening forms.


COVID-19 Vaccine Clinics on Tuesday, December 7 include:
  • Owasco Elementary – 9:00 a.m. – 10:00 a.m.
  • Herman Ave Elementary – 11:00 a.m. – 12:00 p.m.
  • Auburn Junior High – 1:00 p.m. – 2:00 p.m.  

COVID-19 Vaccine Clinics on Thursday, December 9 include:
  • Casey Park Elementary – 9:00 a.m. – 10:00 a.m.
  • Genesee Elementary – 11:00 a.m. – 12:00 p.m.
  • Seward Elementary – 1:00 p.m. – 2:00 p.m.

For students receiving the SECOND DOSE at the December 7 or December 9 clinics: Parents/guardians will only need to fill out another medical screening form (click here to download form) for their child and return it to the main office or health office of your child's school prior to the start of the second dose clinic. Your child will not receive the second dose of the COVID-19 vaccine unless we receive the medical screening form.


For students receiving the FIRST DOSE at the December 7 or December 9 clinics: In order for your child to receive the first dose of the COVID-19 vaccine at these clinics, you will need to sign up AND fill out the minor consent form and medical screening form for each child and return them to the main office or health office of your child’s school prior to the start of the clinic. Your child will not receive the first dose of the COVID-19 vaccine unless we receive both forms. Please note, the second dose will not be offered at the school due to school being closed for the holidays. To receive the second dose, you will need to bring your child to a COVID-19 vaccine clinic at the Finger Lakes Mall or seek an appointment with your provider or at the state fairgrounds.

Dear Auburn Families,

With the U.S. Food and Drug Administration approving the emergency use authorization for the Pfizer-BioNTech COVID-19 Vaccine for children ages 5 through 11, the Cayuga County Health Department is partnering with local school districts to hold vaccine clinics for this age group. The Auburn Enlarged City School District is not mandating the COVID-19 vaccine for students; the district is simply providing opportunities for students to get vaccinated with the consent of parents/guardians.
 
The Cayuga County Health Department will be holding COVID-19 vaccine clinics in AECSD school buildings on November 16 and November 18 for students whose parents/guardians give consent. If desired, parents/guardians may be present while the vaccine is being administered to their child(ren).
 
COVID-19 Vaccine Clinics on Tuesday, November 16* include:
  • Owasco Elementary – 9:00 a.m. – 10:00 a.m.
  • Herman Ave Elementary – 11:00 a.m. – 12:00 p.m.
  • Auburn Junior High – 1:00 p.m. – 2:30 p.m.  
*Second dose clinics will be held on December 7 at the same times at each location.
 
COVID-19 Vaccine Clinics on Thursday, November 18** include:
  • Casey Park Elementary – 9:00 a.m. – 10:00 a.m.
  • Genesee Elementary – 11:00 a.m. – 12:00 p.m.

If you would like to sign your child(ren) ages 5 through 11 up to receive the Pfizer/BioNTech vaccine, please click on the sign-up link below and fill out the form. You must complete a separate form for each child you are signing up to receive the vaccine. All sign-ups must be submitted no later than Monday, November 15 at 8:00 a.m. for the November 16 clinic and Wednesday, November 17 at 8:00 a.m. for the November 18 clinic.

In addition to signing up, please fill out the minor consent form (click here) and medical screening form (click here) for each child and return them to the main office of your child(ren)’s school. All forms must be received no later than Monday, November 15 at 12:00 p.m. for the November 16 clinic and Wednesday, November 17 at 12:00 p.m. for the November 18 clinic. Please note, only one consent form is required to cover both doses of the vaccine.
